4. Patient Mobility and Positioning

Helping patients move safely prevents complications such as pressure ulcers, muscle atrophy, and deep vein thrombosis. Skills include:

  • Turning and Positioning: Regularly changing a patient's position to maintain skin integrity.
  • Transfer Techniques: Safely moving a patient from bed to chair using correct body mechanics or assistive devices like a gait belt or Hoyer lift.
  • Assisting with Ambulation: Helping patients walk to regain independence while ensuring they do not fall.

5. Wound Care and Dressing Changes

Nurses play a critical role in wound management. This involves assessing wounds for signs of infection, cleaning the wound site using aseptic technique, and applying sterile dressings. Documenting the size, color, drainage, and odor of a wound is essential for tracking the healing process.

6. Documentation and Communication

If it wasn't documented, it wasn't done. Accurate, objective, and timely documentation is a legal and clinical requirement. Nurses must also master the skill of "handover" communicationusing frameworks like SBAR (Situation, Background, Assessment, Recommendation) to relay critical patient information clearly to other healthcare team members.
