I, John A. Doe, Employee ID 123456, currently serving in the Marketing Department of XYZ Corporation, acknowledge receipt of the nomination dated 5May2026 for the position of Committee Member Corporate Social Responsibility. I hereby declare that I do not wish to be considered for this role. My decision is based on personal commitments that would prevent me from fulfilling the duties effectively. In most cases the declaration is binding once accepted by the appointing authority. If you wish to withdraw your optout, you must submit a separate written request and obtain explicit approval.
Only if the governing policy or the specific nomination letter states that a reason is required. Otherwise, you may simply state your decision without elaboration.

Many modern organisations accept electronic signatures that comply with local esignature legislation (e.g., eIDAS in the EU, ESIGN Act in the USA). Verify with your HR or legal department before using a digital signature.
