Overview

The Family Center for Children and Youth with Special Health Care Needs (SHCN) is a multidisciplinary hub designed to support families navigating the complex medical, educational, and social landscapes that accompany chronic or disabling conditions. Whether a child is living with a genetic disorder, a developmental disability, a serious physical illness, or a combination of challenges, our mission is to provide coordinated, familycentered care that promotes health, independence, and quality of life.

Families who access the Center benefit from a single location where health professionals, therapists, educators, and social workers collaborate to develop individualized plans. We recognize that each childs needs are unique, and that the strengths of the family system are essential to successful outcomes.

Core Services

Comprehensive Assessment

Our intake process begins with a thorough evaluation covering medical history, developmental milestones, functional abilities, and family dynamics. The assessment team includes a pediatrician, a developmental specialist, a psychologist, and a social worker. The goal is to identify strengths, barriers, and immediate priorities.

Individualized Care Plans

Based on the assessment, a personalized Care Plan is crafted. The plan outlines shortterm goals (e.g., medication titration, speech therapy scheduling) and longterm objectives (e.g., school transition, transition to adult services). Families are active participants; they review and adjust the plan at regular intervals.

Medical Coordination

  • Specialist referrals and appointment management.
  • Medication reconciliation and monitoring.
  • Telehealth visits for families living far from the Center.

Therapeutic Services

Licensed therapists provide inperson and virtual sessions in:

  • Physical therapy enhancing mobility and strength.
  • Occupational therapy fostering independence in daily tasks.
  • Speechlanguage pathology improving communication and feeding skills.
  • Behavioral therapy addressing anxiety, aggression, or selfregulation.

Educational Advocacy

Our education liaison assists families with Individualized Education Programs (IEPs), 504 Plans, and schoolbased health services. We conduct school visits, coach parents on legal rights, and help coordinate accommodations.

Family Support & Counseling

Caring for a child with SHCN can be emotionally taxing. Licensed counselors offer individual, couple, and group therapy. Support groups connect families with peers who share similar experiences, reducing isolation and providing practical tips.

Respite and Community Navigation

Shortterm respite care is arranged through trusted partners, giving caregivers needed breaks. Additionally, our case managers help families locate community resources such as transportation vouchers, nutrition programs, and adaptive equipment grants.
