Ileostomy Nutrition Guidelines

Practical advice to help you feel your best after ileostomy surgery.

Why Nutrition Matters

After an ileostomy, the small intestine directly empties into an external pouch. This change affects fluid balance, electrolyte levels, and nutrient absorption. A wellplanned diet can:

  • Prevent dehydration and electrolyte imbalances.
  • Reduce the risk of blockage or stoma obstruction.
  • Promote wound healing and overall energy.
  • Help you maintain a healthy weight.

General Principles

  • Hydration is priority #1. Aim for 23L (812 cups) of fluid daily, unless your doctor advises otherwise.
  • Eat small, frequent meals. Sixtoeight meals a day help the ileostomy handle the volume.
  • Chew thoroughly. Proper chewing reduces the chance of undigested pieces causing blockage.
  • Balance macronutrients. Include protein, healthy fats, and complex carbs at each meal.
  • Monitor fiber. Some fiber is beneficial, but too much can cause blockages. Choose soluble fiber first.

Fluid & Electrolyte Management

Because a large portion of water and salts are absorbed in the colon, an ileostomy can lead to significant losses.

How Much to Drink

  • Water, herbal teas, clear broths, and diluted fruit juices.
  • Avoid excessive caffeine and alcohol; they increase fluid loss.

Electrolyte Boosters

Consider drinks that contain sodium, potassium, and magnesium, such as oral rehydration solutions, sports drinks (lowsugar), or homemade electrolyte water (add a pinch of salt and a squeeze of lemon).

Tip: Place a glass of water beside every meal and snack to remind yourself to sip regularly.

Protein

Protein supports tissue repair and immune function. Aim for 1.01.5g of protein per kilogram of body weight daily.

  • Lean meats (chicken, turkey, lean beef)
  • Fish and seafood
  • Eggs
  • Lowfat dairy (Greek yogurt, cottage cheese)
  • Plantbased sources (tofu, tempeh, lentils start with small portions and monitor tolerance)
Include a protein source in every snack (e.g., a hardboiled egg or a slice of cheese).

Carbohydrates

Complex carbs provide steady energy and help bulk stool.

  • Wholegrain breads and cereals (choose lowfiber options initially)
  • White rice, couscous, quinoa
  • Mashed potatoes or sweet potatoes (peeled)
  • Cooked carrots, zucchini, and peeled apples

Limit highly processed sugary foods, which can speed up output and cause irritation.

Fats

Healthy fats aid in calorie intake and vitamin absorption.

  • Olive oil, avocado oil
  • Avocado (in moderate amounts)
  • Nut butters (smooth, without large pieces)
  • Fatty fish salmon, mackerel (rich in omega3)

Avoid large fried foods; they can be hard to digest and may increase output.

Fiber The Right Kind and Amount

Fiber is divided into two types:

Soluble Fiber (good for ileostomy)

Absorbs water, forms a gel, and slows digestion. Examples: oats, peeled apples, bananas, peeled carrots, and psyllium husk (start with teaspoon mixed into liquids).

Insoluble Fiber (use with caution)

Can add bulk and cause blockages if taken in large amounts. Examples: whole nuts, seeds, popcorn, raw corn, and unpeeled vegetables.

Increase fiber slowly (12g per week) while observing how your output changes.

Vitamins & Minerals

Because absorption may be altered, pay special attention to:

  • Vitamin B12 Consider a supplement or fortified foods if you have a short ileum.
  • Calcium & Vitamin D Dairy or fortified alternatives plus safe sun exposure.
  • Iron Lean red meat, beans (wellcooked), and ironrich cereals.
  • Magnesium & Potassium Bananas, oranges, potatoes, and leafy greens (cooked and cut small).

Ask your healthcare provider about blood tests to tailor supplementation.

Foods to Approach Carefully

  • Highsugar beverages (can increase output)
  • Carbonated drinks (may cause gas and discomfort)
  • Spicy foods (if they irritate the skin around the stoma)
  • Cruciferous vegetables (broccoli, cauliflower) can cause gas; cook well and start with small portions.
  • Nut skins, whole seeds, popcorn risk of obstruction.

Sample OneDay Meal Plan

Breakfast

cup oatmeal cooked with water, stirred with 1Tbsp smooth almond butter, topped with sliced banana; 1 cup fortified orange juice.

MidMorning Snack

Greek yogurt (plain, lowfat) mixed with 1Tbsp honey and a small handful of soft, peeled fruit (e.g., canned peaches).

Lunch

Grilled chicken breast (3oz) with a drizzle of olive oil, soft cooked carrots and peeled zucchini; cup white rice; 1 cup water with a pinch of salt.

Afternoon Snack

Smooth peanut butter (1Tbsp) on a slice of white toast; a small glass of electrolyte drink.

Dinner

Baked salmon (3oz) with mashed potatoes (no skins); steamed peeled green beans; 1 cup herbal tea (caffeinefree).

Evening Snack

Small bowl of lowfat cottage cheese with a drizzle of maple syrup.

Throughout the day, sip water between meals to reach the 23L goal.

Monitoring & Adjusting

Keep a food and output diary for at least two weeks. Note:

  • What you ate and when.
  • Volume and consistency of stoma output.
  • Any abdominal pain, cramping, or blockage signs.
  • Hydration status (color of urine, frequency).

Share the diary with your dietitian or surgeon to finetune your plan.

When to Seek Help

  • Persistent dehydration despite fluid intake.
  • Severe or sudden change in output (very watery or chunky).
  • Abdominal pain, bloating, or vomiting.
  • Unexplained weight loss or fatigue.

These symptoms may indicate a blockage, infection, or nutritional deficiency and require professional evaluation.
