Metabolic Syndrome (MetS)

What Is Metabolic Syndrome?

Metabolic syndrome, often abbreviated as MetS, is a cluster of interrelated metabolic abnormalities that together increase the risk of cardiovascular disease (CVD), type2 diabetes, and premature death. The syndrome is not a single disease but rather a constellation of risk factors that share common pathophysiological mechanisms, most notably insulin resistance and chronic lowgrade inflammation.

Diagnostic Criteria

Several professional organizations have defined MetS using slightly different thresholds, but the most widely accepted criteria come from the National Cholesterol Education Program Adult Treatment PanelIII (NCEPATPIII). According to NCEPATPIII, a person is diagnosed with MetS when three or more of the following five conditions are present:

  • Abdominal obesity: waist circumference 102cm (40in) in men or 88cm (35in) in women.
  • Elevated triglycerides: 150mg/dL (1.7mmol/L) or treatment for hypertriglyceridemia.
  • Reduced HDLcholesterol: <40mg/dL (<1.0mmol/L) in men, <50mg/dL (<1.3mmol/L) in women, or therapy to raise HDL.
  • High blood pressure: systolic 130mmHg or diastolic 85mmHg, or use of antihypertensive medication.
  • Impaired fasting glucose: fasting plasma glucose 100mg/dL (5.6mmol/L) or treatment for elevated glucose.

Other groups, such as the International Diabetes Federation (IDF), require abdominal obesity as a mandatory component and apply ethnicityspecific waistcircumference thresholds.

Causes & Risk Factors

MetS results from a complex interaction of genetic, environmental, and behavioural factors.

Insulin Resistance

Central to the syndrome, insulin resistance impairs glucose uptake in muscle and adipose tissue, prompting the pancreas to secrete more insulin. Chronic hyperinsulinemia drives triglyceride synthesis, lowers HDL, and contributes to hypertension.

Visceral Adiposity

Fat stored around the abdomen is metabolically active, releasing free fatty acids, adipokines, and proinflammatory cytokines (e.g., TNF, IL6). These mediators exacerbate insulin resistance and endothelial dysfunction.

Genetics & Ethnicity

Family history of diabetes or CVD increases susceptibility. Certain ethnic groupsSouth Asians, Hispanics, AfricanAmericanstend to develop MetS at lower bodymass indexes.

Lifestyle Factors

Other Contributing Conditions

Polycystic ovary syndrome, certain endocrine disorders (e.g., Cushings syndrome), and use of some medications (e.g., glucocorticoids, antipsychotics) can precipitate MetS.

Health Consequences

When left untreated, metabolic syndrome markedly elevates longterm health risks.

  • Cardiovascular disease: a 23fold increase in coronary heart disease and stroke.
  • Type2 diabetes: individuals with MetS are 5 times more likely to develop diabetes.
  • Nonalcoholic fatty liver disease (NAFLD): hepatic steatosis can progress to cirrhosis.
  • Kidney disease: insulin resistance and hypertension accelerate chronic kidney disease.
  • Cancer: higher incidence of colorectal, breast, and pancreatic cancers has been reported.

Management Strategies

Effective treatment targets each component of the syndrome, with lifestyle modification forming the cornerstone of therapy.

Dietary Changes

Adopt a Mediterraneanstyle or DASH diet rich in vegetables, fruits, whole grains, legumes, nuts, and lean protein. Limit added sugars, refined carbohydrates, saturated fats, and transfatty acids.

Physical Activity

Engage in at least 150minutes of moderateintensity aerobic exercise per week (e.g., brisk walking, cycling) combined with twiceweekly resistance training.

Weight Management

Even modest weight loss (510% of body weight) can improve insulin sensitivity, lower blood pressure, and favorably modify lipid levels.

Pharmacologic Therapy

Medications may be required to achieve target levels:

  • Antihypertensives: ACE inhibitors, ARBs, thiazide diuretics.
  • Lipidlowering agents: statins for LDLC reduction; fibrates or omega3 fatty acids for triglycerides.
  • Glucoselowering drugs: metformin is often used when fasting glucose is elevated; newer agents (SGLT2 inhibitors, GLP1 receptor agonists) have cardiovascular benefits.

Monitoring

Regular followup every 36months includes measuring waist circumference, blood pressure, fasting glucose, and a lipid panel.

Preventing metabolic syndrome starts early. Encourage families to:

  • Choose whole, minimally processed foods.
  • Stay activeincorporate walking or biking into daily routines.
  • Limit sugary drinks and excessive alcohol.
  • Prioritize sleep (79hours/night) and stress management.
  • Undergo regular health screenings, especially if there is a family history of diabetes or heart disease.
