Nutrition Research Priorities in Malaysia for the 12th Malaysia Plan (20212025)

The 12th Malaysia Plan (Plan12) recognises that nutrition is a cornerstone of human health, economic productivity, and national development. Malaysia is experiencing a rapid nutrition transition, with rising prevalence of overweight, obesity, dietrelated noncommunicable diseases (NCDs), and persisting micronutrient deficiencies among vulnerable groups. To address these challenges, the Ministry of Health (MOH), Ministry of Higher Education, and other stakeholders have identified a set of research priorities that will guide funding, capacity building, and policy formulation through 2025.

1. Context and Rationale

Key statistics driving the agenda:

  • Over 30% of adults are obese and more than 47% are overweight (NHMS 2020).
  • Micronutrient deficiency affects 22% of children under five (iron, vitaminA, iodine).
  • Dietary patterns show high intake of sugarsweetened beverages, processed meats, and low fruitvegetable consumption.
  • Food insecurity remains a concern in rural and lowincome urban communities.

These trends underscore the need for evidencebased interventions that are culturally appropriate, costeffective, and scalable.

2. Overarching Goals of Nutrition Research in Plan12

  1. Improve population nutritional status by reducing undernutrition, overnutrition, and dietrelated NCDs.
  2. Strengthen the evidence base for nutrition policies, programmes, and regulations.
  3. Build national research capacity in nutrition science, epidemiology, and implementation research.
  4. Promote interdisciplinary collaboration among health, agriculture, education, and industry sectors.

3. Priority Research Themes

3.1. Food Systems and Sustainable Nutrition

Investigate the links between agricultural practices, food processing, distribution, and dietary outcomes. Focus areas include:

  • Assessing the nutritional quality and environmental footprint of locally produced foods.
  • Developing strategies to promote production and consumption of nutrientdense, climateresilient crops.
  • Evaluating supplychain interventions that improve access to affordable healthy foods in periurban and rural areas.

3.2. Dietary Patterns and NCD Prevention

Largescale cohort and casecontrol studies that map Malaysian dietary patterns against cardiometabolic risk factors. Research gaps include:

  • Longitudinal impact of traditional Malay, Chinese, and Indian diets on blood pressure, lipid profiles, and glycaemic control.
  • Effectiveness of communitybased lowsalt and lowsugar interventions.
  • Role of gut microbiota in mediating dietrelated disease risk among Malaysians.

3.3. Micronutrient Deficiency and Fortification

Targeted studies to refine fortification programmes and monitor outcomes:

  • Impact assessment of ironfolic acid (IFA) supplementation among adolescent girls and pregnant women.
  • Bioavailability studies of fortified rice, cooking oil, and condiments in realworld settings.
  • Exploration of biofortified crops (e.g., vitaminArich cassava) suited to Malaysian agroecologies.

3.4. EarlyLife Nutrition

Critical window interventions from preconception to age2years:

  • Maternal nutrition and epigenetic markers linked to offspring growth trajectories.
  • Effectiveness of exclusive breastfeeding promotion and complementary feeding guidelines.
  • Nutrition-sensitive parenting programmes that integrate behaviour change communication.

3.5. Nutrition Literacy and Behaviour Change

Understanding knowledgeattitudepractice (KAP) gaps and designing culturally resonant education tools:

  • Digital health interventions (mobile apps, SMS reminders) for diet monitoring.
  • Schoolbased nutrition curricula that incorporate local food cultures.
  • Evaluation of massmedia campaigns on sugarsweetened beverage consumption.

3.6. Food Policy, Regulation, and Economics

Evidence to support policy decisions:

  • Costeffectiveness of taxation on sugary drinks versus subsidies for fruits and vegetables.
  • Compliance monitoring of frontofpack nutrition labelling and health claims.
  • Economic modelling of the health burden averted through nutritionfocused public policies.

3.7. Indigenous and Rural Nutrition

Specific studies for Orang Asli, Sabah, and Sarawak communities:

  • Documentation of traditional food systems and their nutritional contributions.
  • Assessment of food security determinants in remote villages.
  • Cocreation of interventions that respect cultural practices while improving nutrient intake.

4. Methodological Approaches

Plan12 places emphasis on robust, mixedmethods designs:

  • Largescale surveillance Continuation and expansion of the National Health and Morbidity Survey (NHMS) with nutrition modules.
  • Randomised controlled trials Testing efficacy of specific dietary interventions in community and clinical settings.
  • Implementation science Translating proven interventions into routine practice, analysing barriers and facilitators.
  • Big data analytics Leveraging electronic health records and food purchase data for trend analysis.
  • Participatory research Engaging local stakeholders to codesign solutions, ensuring relevance and sustainability.

5. Capacity Building and Collaboration

Key actions to strengthen the research ecosystem:

  1. Establish a National Nutrition Research Network linking universities, research institutes, and government agencies.
  2. Provide competitive grants for earlycareer nutrition scientists, with a focus on interdisciplinary projects.
  3. Develop a centralized data repository for nutrition surveillance, accessible to accredited researchers.
  4. Organise annual workshops on advanced methodologies (e.g., metabolomics, GIS mapping of food environments).
  5. Encourage publicprivate partnerships for food technology, fortification, and healthtech innovations.

6. Expected Outcomes by 2025

  • Reduction of national obesity prevalence from 30% to 25%.
  • Decrease in irondeficiency anaemia among women of reproductive age by 15%.
  • Implementation of mandatory frontofpack labelling across all prepackaged foods.
  • Scaling of at least three evidencebased community nutrition programmes to 50% of districts.
  • Publication of 120 peerreviewed articles on Malaysian nutrition research, with increased citations in international journals.

7. Monitoring and Evaluation

A dedicated Nutrition Research Monitoring Unit will track progress using SMART indicators (Specific, Measurable, Achievable, Relevant, Timebound). Annual reports will be submitted to the Prime Ministers Office and made publicly available.
