About the Board
The Oklahoma State Board of Medical Licensure & Supervision (OSBMLS) is the state agency responsible for protecting the publics health and safety by ensuring that physicians, physician assistants, and other healthcare professionals meet the standards set by Oklahoma law. The Board reviews applications, issues and renews licenses, investigates complaints, and takes disciplinary action when necessary.
The Board consists of 15 members: six physicians, three physician assistants, three public members, two representatives appointed by the Oklahoma Department of Health, and the State Health Officer, who serves as an exofficio member. This blend of professional and public input helps balance expertise with consumer protection.
Licensing Process
All healthcare practitioners who wish to practice in Oklahoma must be licensed by the Board. The general steps are:
- Create an account on the OSBMLS online portal.
- Complete the appropriate application form (MD, DO, PA, etc.).
- Submit required documentation, including education transcripts, verification of training, and proof of citizenship or legal residency.
- Pay the applicable application fee (fees vary by profession).
- Undergo a criminal background check performed by the Oklahoma State Bureau of Investigation.
- Provide evidence of passing the United States Medical Licensing Examination (USMLE) or COMLEXUSA for osteopathic physicians, and the relevant national exam for physician assistants.
- Await review. The Board typically processes applications within 3045 days if all materials are complete.
Eligibility & Requirements
Physicians (MD/DO)
- Graduation from a medical school accredited by the LCME (MD) or COCA (DO).
- Completion of at least one year of postgraduate training (internship or residency).
- Passing scores on USMLE Step 1, Step 2 CK, and Step 3, or COMLEXUSA equivalents.
- Proof of good moral characterno felony convictions related to health care.
Physician Assistants (PA)
- Graduation from an Accreditation Review Commission on Education for the Physician Assistant (ARCPA) accredited program.
- Successful completion of the Physician Assistant National Certifying Exam (PANCE).
- Completion of at least 2,000 hours of supervised clinical practice.
Other Licensed Professionals
The Board also oversees limited permits for nonphysician practitioners such as clinical psychologists, pharmacists, and anesthesia technicians. Each profession has its own specific educational and examination requirements, all of which are detailed on the Boards website.
License Renewal & Continuing Education
Oklahoma licenses are valid for two years. Renewal must be completed online before the expiration date; late renewals incur a $50 penalty.
All licensees must meet continuing medical education (CME) requirements:

Complaints, Investigations, & Discipline
Any individual may file a complaint against a licensed professional. Complaints are reviewed by the Boards Investigative Committee and may result in:
- Informal counseling or warning.
- Formal reprimand.
- Probation with specific conditions.
- Suspension or revocation of the license.
- Fines up to $5,000 per violation.
All investigations are conducted confidentially, and complainants can request anonymity. The Board publishes disciplinary actions on its public website to maintain transparency.
