Identify the congregation (often by its legal name) and the pastor by full legal name.
State the exact title (e.g., Senior Pastor, Associate Pastor of Youth) and a brief description of the role.
Specify the start date and whether the appointment is for a fixed term (e.g., three years) or indefinite, with any renewal procedures.
Outline core responsibilities such as preaching, pastoral care, administration, counseling, and any specific ministry expectations.
Describe to whom the pastor reports (e.g., Board of Elders, Church Council) and the frequency of performance reviews.
Include expectations regarding confidentiality of church matters, moral conduct, and adherence to the churchs statement of faith.
Detail the process for either party to end the relationship, required notice periods, and any severance or payout provisions.
State how disagreements will be addressedoften through mediation or a defined arbitration process.
Both the pastor and an authorized church representative must sign and date the letter. Including a witness line can add an extra layer of formality.

Either party may terminate this agreement with 90 days written notice. In the event of termination without cause, the church will provide a severance equal to one months salary for each completed year of service.
Any dispute arising out of this agreement shall be resolved first through mediation; if unsuccessful, arbitration in Cook County, Illinois, will be the final step.
