Physical Exam Parameters Useful in the Assessment of Nutritional Status

Assessment of nutrition goes beyond laboratory tests. A thorough physical examination can reveal subtle signs of deficiency, excess, or malabsorption that may otherwise be missed. The following parameters are routinely examined by clinicians to gauge a patients nutritional status.

1. Anthropometric Measurements

Body Mass Index (BMI)

BMI = weight (kg) height (m). It provides a quick categorization of underweight, normal weight, overweight, and obesity. While useful for population screening, BMI does not differentiate between muscle and fat mass.

MidUpper Arm Circumference (MUAC)

Measured at the midpoint between the acromion and olecranon. Low MUAC (<23cm in adults) suggests proteinenergy wasting, especially in the elderly or in chronic disease.

Skinfold Thickness

Triceps, subscapular, and suprailiac skinfolds estimate subcutaneous fat stores. Equations such as the JacksonPollock formula convert measurements to body fat percentage.

WaisttoHip Ratio (WHR)

Elevated WHR (>0.90 in men, >0.85 in women) indicates central adiposity, which correlates with metabolic risk independent of BMI.

2. General Appearance

  • Muscle mass: Visual thinning of the clavicular and deltoid muscles, or prominence of bony landmarks (e.g., ribs, spine) points to muscle wasting.
  • Subcutaneous fat: Loss of fat pads over the triceps, elbows, and buttocks signals chronic energy deficiency.
  • Hair & skin: Brittle hair, hair loss, dry scaly skin, or hyperpigmentation may reflect protein, zinc, or essential fatty acid deficiencies.
  • Oral cavity: Glossitis, angular cheilitis, atrophic oral mucosa, and loss of papillae suggest Bvitamin or iron deficiency.

3. Specific Physical Signs

Edema

Pitting edema, especially in the lower extremities, often accompanies hypoalbuminemia or severe protein deficiency.

Acrodermatitis EnteropathicaLike Rash

Periorificial and acral dermatitis can indicate zinc deficiency.

Beriberi Signs

Peripheral neuropathy, muscle weakness, and edema are classic for thiamine (vitamin B1) deficiency.

Scurvy Manifestations

Gingival bleeding, perifollicular hemorrhage, and corkscrew hairs signal vitamin C deficiency.

Night Blindness & Xerophthalmia

Early ocular signs of vitamin A deficiency; assess conjunctival and corneal health.

4. Functional Tests

  • Handgrip Strength: Measured with a dynamometer. Low grip strength (<30kg men, <20kg women) is a robust marker of sarcopenia and poor nutritional status.
  • Timed UpandGo (TUG): Slower performance may reflect reduced muscle mass and energy reserves.
  • Gait Speed: Walking slower than 0.8m/s correlates with frailty and undernutrition.

5. Laboratory Correlates (Brief Overview)

Physical findings should be interpreted alongside basic labs: serum albumin, prealbumin, transferrin, complete blood count, and micronutrient panels when specific deficiencies are suspected.

6. Putting It All Together

Effective nutritional assessment is multidimensional. A clinician should:

  1. Record anthropometric data (BMI, MUAC, skinfolds, WHR).
  2. Observe general appearance for muscle and fat loss.
  3. Identify characteristic skin, hair, oral, and ocular signs.
  4. Perform functional tests when possible.
  5. Correlate findings with laboratory results and the patients medical history.

By integrating these physical exam parameters, health professionals can detect malnutrition early, monitor response to intervention, and guide targeted nutritional therapy.
