Guidelines for Gastric Bypass & Sleeve Gastrectomy Patients

Why an Eating Plan Matters

After bariatric surgery the stomachs size, shape and function are dramatically altered. The food you choose, the amount you eat, and the speed at which you consume it directly affect healing, nutrient absorption, and longterm weight loss. A structured eating plan helps you:

  • Prevent dumping and nausea
  • Promote proper wound healing
  • Avoid vitamin and mineral deficiencies
  • Establish new, sustainable habits

General Principles (Both Procedures)

These rules apply to the gastric bypass and sleeve gastrectomy unless otherwise noted:

  • Small portions cup ( cup for liquids) per bite.
  • Chew thoroughly 2030 times per bite.
  • Eat slowly 2030 minutes for a full meal.
  • Stay upright 3060min after eating.
  • Hydrate wisely sip water between meals, not with food.
  • No carbonated drinks for the first 6weeks.

Stage 1 Clear Liquids (Day13)

Goal: Prevent dehydration while allowing the stomach to rest.

  • Water, herbal tea, broth (lowsodium), sugarfree gelatin.
  • Avoid caffeine, alcohol, and acidic juices.
  • Consume 12cups (816oz) every 2hours; aim for 4864oz per day.
Tip: Keep a log of fluid intake; dehydration is a common early complication.

Stage 2 Full Liquids (Day47)

Goal: Introduce protein while staying liquid.

  • Protein shakes (lowsugar, whey or soy), strained soups, lowfat yogurt, cottage cheese.

  • Soft cooked fish, ground poultry, scrambled eggs, lowfat cheese, soft fruits (banana, melon), wellcooked vegetables.
  • Protein: 6080g per day roughly 23 servings of protein.
  • Continue to limit sugar, fried foods, and tough fibrous items (e.g., raw carrots, nuts).

Stage 5 LongTerm Maintenance (After 6Weeks)

Goal: Sustain weight loss, meet vitamin/mineral needs, and enjoy a varied diet.

Protein

  • Minimum 60g/day (1.52servings per meal).
  • Prioritize lean meats, fish, eggs, lowfat dairy, legumes, and protein powders.

Veggies & Fruit

  • At least 5 servings of nonstarchy vegetables per day.
  • One whole fruit per day (avoid large fruit bowls).

Carbohydrates

  • Choose whole grains, legumes, and starchy veg in controlled portions (cup cooked).
  • Avoid sugary drinks, pastries, and candy.

Fats

  • Emphasize healthy fats olive oil, avocado, nuts (in cup portions).
  • Limit fried foods and butter.

Supplements

Both surgeries create malabsorption risks. Most surgeons recommend a lifelong regimen of:

  • Multivitamin with minerals
  • Calcium citrate 1,2001,500mg (split doses)
  • Vitamin D 2,0005,000IU
  • Vitamin B12 (injectable or sublingual) 5001,000g weekly
  • Iron (especially for bypass) 4560mg elemental iron
