Practice Nurse Performance Review

Why Review a Practice Nurse?

Practice nurses are the backbone of primary care. They provide immunisations, chronic disease monitoring, health promotion and urgent care. A structured performance review helps to:

  • Ensure patient safety and highquality care.
  • Identify strengths and areas for improvement.
  • Align daily work with the practices strategic goals.
  • Motivate staff through recognition and career planning.
  • Document performance for regulatory compliance.

The Review Process

A welldesigned review cycle typically follows these steps:

  1. Preparation Collect data (clinical audit results, patient surveys, peer observations) and set the agenda.
  2. SelfAssessment The nurse reflects on achievements, challenges and learning needs.
  3. Manager Meeting Discuss performance against standards, provide balanced feedback and agree on objectives.
  4. Action Planning Define measurable goals, required training, and timelines.
  5. Followup Regular checkins (quarterly or monthly) to monitor progress.
Tip: Keep the review conversation collaborative, not punitive. Use openended questions such as What support would help you achieve your targets?

Key Performance Criteria

Performance should be evaluated against both clinical and professional standards:

Clinical Competence

  • Accuracy of medication administration and vaccinations.
  • Effective chronic disease management (e.g., hypertension, diabetes).
  • Adherence to clinical guidelines and protocols.

Communication & Patient Relations

  • Clear, empathetic communication with patients and families.
  • Effective health education and promotion.
  • Responsiveness to patient concerns and complaints.

Teamwork & Leadership

  • Collaboration with GPs, reception staff, and allied health professionals.
  • Contribution to practice meetings and qualityimprovement projects.
  • Mentoring of junior staff or student nurses.

Professionalism

  • Attendance, punctuality and reliability.
  • Maintenance of confidentiality and data protection.
  • Commitment to continuous learning and revalidation.

Giving Effective Feedback

Feedback should be specific, timely, and balanced:

  • SituationBehaviourImpact (SBI) model describe the situation, the observed behaviour, and its impact on patients or the team.
  • Pair praise with constructive points. Example: Your fluclinic workflow reduced waiting times by 15%. Could we apply a similar approach to the chronic disease review slots?
  • Encourage selfreflection: ask How do you feel that went? before offering suggestions.
Avoid language that sounds like a personal attack. Focus on actions, not the individuals character.

Professional Development & Support

Link performance outcomes to tangible development opportunities:

  • Training courses e.g., Advanced Cardiac Life Support, Motivational Interviewing.
  • Mentoring programmes Pair with an experienced practice nurse or a GP with an interest in education.
  • Study leave Support enrolment in a diploma or MSc in Primary Care Nursing.
  • Reflective practice Encourage use of a learning journal or portfolio.

Document agreed actions in a written appraisal form and review them at the next checkin. Successful completion of development goals should be recognised, for example with a certificate, a small award, or public acknowledgment at staff meetings.
