Radiography Positioning Guide
Radiography positioning is the foundational skill required for diagnostic imaging. Proper positioning ensures that anatomical structures are visualized clearly, accurately, and without distortion, allowing radiologists to make informed diagnostic decisions. This guide outlines the essential principles and techniques for standard radiographic procedures.
Core Principles of Positioning
To produce high-quality images, radiographers must adhere to specific standards regarding patient orientation and beam alignment. The primary goals are to demonstrate the area of interest, minimize magnification, and reduce anatomical superimposition.
- Source-to-Image Distance (SID): Maintaining a standard distance (commonly 40 inches for general radiography and 72 inches for chest imaging) is critical for consistency.
- Central Ray (CR) Alignment: The CR must be directed perpendicular to the anatomy or at a specific angle to eliminate distortion or foreshortening of the image.
- Patient Comfort and Safety: Proper communication and the use of immobilization devices help reduce motion artifacts and ensure patient cooperation.
Common Radiographic Projections
1. Chest Radiography (PA View)
The Posterior-Anterior (PA) chest projection is the standard view for visualizing the lungs, heart, and mediastinum.
- Positioning: The patient stands facing the upright bucky with the chest against the board.
- Alignment: The mid-sagittal plane is centered to the midline of the IR. The shoulders are rolled forward to move the scapulae out of the lung fields.
- Instruction: The patient is asked to take a deep breath and hold it to ensure full inspiration.
2. Hand Radiography (PA View)
This view evaluates the phalanges, metacarpals, and carpals.
- Positioning: The hand is placed flat on the IR with the palm down.
- Alignment: The fingers are slightly spread. The CR is directed to the third metacarpophalangeal joint.
3. Lateral Cervical Spine
Used to evaluate the vertebrae and the prevertebral soft tissues.
- Positioning: The patient is positioned in a true lateral position with the side of interest closer to the IR.
- Alignment: The chin is slightly elevated to prevent superimposition of the mandible over the vertebrae. The beam is typically centered at C4.
The Importance of Image Quality
Even with perfect positioning, image quality relies on the "ALARA" principle (As Low As Reasonably Achievable). By utilizing correct positioning techniques, radiographers avoid the need for repeat exposures, thereby minimizing radiation dose to the patient and optimizing the diagnostic utility of the image.
Troubleshooting Common Errors
Positioning errors are the most common cause of non-diagnostic images. Some frequent challenges include:
- Rotation: If the anatomy appears asymmetrical, the patient is likely rotated. Always ensure the patient's plane is parallel or perpendicular to the IR as required.
- Angulation: If the image appears foreshortened or elongated, check the CR angle relative to the structure of interest.
- Motion Artifacts: Blurring is often caused by patient movement. Clear communication of breathing instructions or the use of short exposure times can mitigate this.
Professional Standards
Every radiographer must be familiar with anatomical landmarks to ensure precise centering. Understanding the relationship between the internal anatomy and the external surface landmarks allows for accurate beam placement, regardless of variations in patient body habitus.
