No single suture is ideal for all tissues or procedures; rather, the surgeon must balance the biological requirements of the wound with the mechanical properties of the material. Effective selection relies on an understanding of tissue healing dynamics, suture degradation profiles, and handling characteristics.
1. Absorption Profile
The primary distinction in suture material is whether it is absorbable or non-absorbable.
- Absorbable Sutures: These are indicated when the suture is only required to provide temporary support until the tissue regains sufficient tensile strength. Examples include polyglactin 910 (Vicryl) or polydioxanone (PDS). These are preferred for internal tissues, buried sutures, and pediatric procedures where suture removal would be difficult.

Every suture material elicits some degree of inflammatory response. When selecting a material, surgeons must consider the sensitivity of the target tissue.
- Synthetic Materials: Generally cause the least inflammatory reaction as they are broken down through predictable hydrolysis.
- Natural Materials: Materials such as chromic gut or silk elicit a more pronounced inflammatory response as the body breaks them down through proteolysis, which may be counterproductive in highly sensitive areas like the eyes or certain cosmetic repairs.
4. Tensile Strength and Rate of Loss
The suture must maintain its tensile strength for a duration that matches the wound's healing process. If the suture loses its strength before the wound has adequately bridged the gap, dehiscence may occur. Conversely, choosing a suture that retains strength for too long in a fast-healing tissue may result in a permanent foreign body reaction or localized irritation.
5. Knot Security and Handling
In clinical practice, the "feel" of a suture is critical. A suture that is too stiff may "memory" its original shape, making it difficult to tie securely. High-friction materials (like braided sutures) generally offer excellent knot security, while slippery monofilaments like polypropylene require a greater number of square knots to prevent slippage.
