Certificate Replacement Request Form
If you have lost, damaged, or need a corrected version of a qualification certificate issued by University College Birmingham (UCB), you can request a replacement using the official online form. Who Can Apply?
- Former students who have successfully completed a degree, diploma, or professional qualification at UCB.
- Current students who have completed a module and received a provisional certificate that later requires replacement.
- Alumni applying for a duplicate of a previously issued certificate that has been misplaced or damaged.
When to Use the Replacement Form
You should complete the replacement request form in the following circumstances:
- Original certificate is lost or stolen.
- Certificate is damaged beyond readability (e.g., water damage, fire).
- There is an error on the original (misspelled name, wrong grade, etc.).
- You need an extra copy for employment, further study, or professional registration.

- Gather Required Information
- Full name as it appears on the original certificate.
- Student ID or enrolment number.
- Course title, qualification level and graduation year.
- Reason for replacement (loss, damage, correction, extra copy).
- Prepare Supporting Documents
- Copy of a valid photo ID (passport or driving licence).
- If the certificate was stolen, a police report or reference number.
- If you are requesting a correction, evidence of the correct details (e.g., birth certificate).
- Visit the Online Form
Go to the official UCB replacement form at https://www.ucb.ac.uk/certificates/replacement.
- Complete the Form
- Enter all personal details exactly as they appear on the university records.
- Select the type of certificate you need (Bachelors, HND, Short Course, etc.).
- Upload scanned copies of supporting documents (PDF or JPEG, max 5MB each).
- Pay the Replacement Fee
The standard fee is 25+VAT for a standard replacement. Faster processing (within 5working days) costs an additional 15. Payment must be made by credit/debit card or PayPal at the end of the form.
- Submit the Request
After reviewing the entered information, click the Submit Request button. You will receive an automatic acknowledgment email containing a reference number.
- Processing & Delivery
- Standard processing takes 1012working days. Express service is 5working days.
- Certificates are mailed to the address you confirm on the form. If you need the certificate sent to an alternative address, indicate it clearly.
- Delivery is via Royal Mail tracked service for standard requests and a courier for express requests.
Frequently Asked Questions
Can I request more than one replacement?
Yes. Each additional copy requires a separate fee. The form allows you to request up to three copies in a single submission.
What if I no longer have my Student ID?
Provide your full name, date of birth, and the year you completed your course. The records team will locate your file using this information.
Will the replacement have the same serial number as the original?
No. The replacement receives a new unique serial number but will display the same qualification details.
Can I collect the certificate in person?
Yes. If you prefer to pick it up at the UCB Campus, select Collect in person on the delivery option page and arrange a suitable time with the Student Records Office.
Is there a deadline for requesting a replacement?
There is no strict deadline, but the longer you wait, the more likely additional verification may be required, which could delay the process.
