Consent for Attending Offline Classes & Adhering to COVID19 Guidelines
As educational institutions resume facetoface teaching, the health and safety of students, faculty, staff, and visitors remain the top priority. By signing this consent, you acknowledge that you have read, understood, and agree to follow the COVID19 protocols outlined below while attending offline classes.
Why a Consent Form is Required
- Ensures that every participant is aware of the measures designed to reduce virus transmission.
- Provides legal clarity for the institution in the event of a health incident.
- Helps create a transparent environment where safety expectations are shared.
Core COVID19 Safety Measures
1. Vaccination and Testing
- All participants must be fully vaccinated against COVID19, or present a valid medical exemption.
- Unvaccinated individuals are required to submit a negative RTPCR or rapid antigen test taken within 72 hours prior to campus entry.
2. Mask Policy
- Medicalgrade masks (e.g., N95, KN95, surgical masks) are mandatory in indoor spaces, lecture halls, laboratories, libraries, and common areas.
- Masks may be removed only when a student is eating or drinking in designated areas that are well ventilated and spaced apart.
3. Physical Distancing
- Classrooms will be arranged to maintain at least 1meter (3feet) between individuals.
- Seating charts will be posted; students must sit in assigned seats.
4. Hand Hygiene
- Handsanitizer dispensers are placed at entry points, exits, and hightraffic zones. Use them frequently.
- Wash hands with soap and water for at least 20 seconds when possible.
5. Ventilation
- All indoor spaces are equipped with upgraded HVAC systems; windows are kept open where feasible.
- Air purifiers with HEPA filters are installed in classrooms that cannot be adequately ventilated.
6. Symptom Monitoring & Reporting
- Daily selfassessment through the institutions health portal is required before campus entry.
- If you develop fever (38C), cough, shortness of breath, loss of taste/smell, or any other COVIDrelated symptoms, stay home, get tested, and notify the health office immediately.
7. Contact Tracing
- All attendees must wear the institutions ID badge equipped with a Bluetooth-based proximity tracker.
- Data will be used solely for contacttracing purposes and will be deleted after 30 days.
8. Cleaning & Disinfection
- Hightouch surfaces (door handles, desks, computer keyboards) are sanitized every 2hours.
- Classrooms are deepcleaned nightly.
Student Responsibilities
- Arrive on time for class with a clean mask and proof of vaccination or a recent negative test.
- Follow all signage and verbal instructions from faculty and staff.
- Refrain from sharing food, drinks, or personal items.
- Respect the designated seating arrangement and do not move without permission.
- Report any violation of the guidelines to campus security or the health office.
Faculty & Staff Responsibilities
- Verify vaccination status or test results for each participant at the start of the session.
- Enforce mask usage and physical distancing throughout the class.
- Leave the room for at least 15 minutes after any student reports symptoms before cleaning staff enters.
- Maintain a log of attendance for contacttracing purposes.
Consequences of NonCompliance
Failure to adhere to the above guidelines may result in:
- Temporary suspension of campus access.
- Mandatory quarantine as per local health authority directives.
- Disciplinary action in accordance with the institutions code of conduct.
- Potential legal liability if negligence leads to an outbreak.
