Overview
Louisianas Medicaid program is financed jointly by the state and the federal government and provides health coverage for eligible lowincome individuals, families, seniors, and people with disabilities. Because Medicaid financing and eligibility are governed by both state law and federal statutes, the program is organized into a series of bill types. Each bill type represents a specific category of services, provider arrangements, or funding mechanisms. Understanding these bill types helps providers submit accurate claims, assists beneficiaries in navigating coverage, and aids policymakers in managing the programs budget.
Core Bill Types
1. Managed Care (MC) Bill Types
Most Louisiana Medicaid beneficiaries receive services through one of the states managed care organizations (MCOs). The MC bill types include:
- MC001 Primary Care Services: Visits to primary care physicians, nurse practitioners, and qualified health centers.
- MC002 Behavioral Health Services: Outpatient mental health, substanceuse treatment, and psychiatric medication management.
- MC003 Hospital & Emergency Services: Inpatient stays, ER visits, and observation services paid to hospitals participating in the MCO network.
- MC004 LongTerm Services & Supports (LTSS): Home health, personal care, and communitybased waiver services.
2. FeeForService (FFS) Bill Types
Providers who are not contracted with an MCO may bill the state directly under feeforservice arrangements. Key FFS bill types are:
- FFS101 Physician Services: Office visits, procedural services, and consultations.
- FFS102 Dental Services: Preventive, restorative, and emergency dental care.
- FFS103 Pharmacy Services: Prescription drugs dispensed under the Medicaid Drug Rebate Program.
- FFS104 Durable Medical Equipment (DME): Supplies such as walkers, oxygen equipment, and prosthetics.
3. Institutional Bill Types
These bill types apply to care delivered in institutional settings such as hospitals, nursing facilities, and rehabilitation centers.
- INST201 Acute Hospital Care: Full inpatient admissions, including room and board, nursing, and ancillary services.
- INST202 Skilled Nursing Facility (SNF): 24hour nursing care, therapy, and related services for qualifying patients.
- INST203 Inpatient Rehabilitation: Intensive physical, occupational, and speech therapy after acute events.
Specialized Bill Types
4. Waiver & State Plan Services
Louisiana operates several waiver programs that provide home and communitybased alternatives to institutional care. Bill types include:
- WAIV301 Home and Community Based Services (HCBS) Waiver: Personal assistance, respite care, and case management.
- WAIV302 FaithBased & Community Organizations: Services delivered by nonprofit agencies under the state plan.
5. Supplemental Services
These bill types cover services that are not core medical care but are essential for health outcomes.
- SUP401 Transportation: Nonemergency medical transportation (NEMT) to and from appointments.
- SUP402 Nutritional Services: Meals provided to eligible seniors and persons with disabilities.
6. Pharmacy Bill Types
Medicaid drug coverage is split between two primary categories:
- PHARM501 Preferred Drug List (PDL) Drugs: Medications that must be dispensed under the states preferred formulary.
- PHARM502 NonPreferred / Specialty Drugs: Highercost medications that require prior authorization.
How Bill Types Are Used
When a provider renders a service, the electronic claim must include the appropriate bill type code. The code tells the Medicaid Management Information System (MMIS) how to process the claim, which rate to apply, and whether any prior authorization is required.
Beneficiaries can also see bill types on their Monthly Benefit Statements. Understanding the categories helps them track what services are covered and identify any gaps.
Recent Changes (20232024)
- Expansion of MC004: The LongTerm Services & Supports bill type now includes additional telehealth visits for homebound patients.
- New PHARM503 COVID19 Therapeutics: A temporary bill type created for monoclonal antibodies and antiviral drugs administered under the emergency response.
- Revised FFS103 Rates: Pharmacy reimbursement rates were adjusted to align with the federal Medicaid Drug Rebate Program inflation factor.
